HealthReach Community Education

Our health and wellness educators are here to help you! We provide our clients with up-to-date information about health and disease managementto help them make informed healthcare decisions and to promote a sense of health and well-being in our community.

In addition to the disease management programs for Diabetes, Respiratory, and Osteoporosis, we also offer a variety of other injury prevention and early detection programs. Examples of these programs include:
