A subsidiary of Exeter Health Resources, Inc., the Postacute Rehabilitation Center at Exeter Healthcare serves the needs of the postacute patient.  Our unit was designed to provide care to patients after a stay in the hospital, usually following an illness or surgery.  You may think of it as a bridge between the hospital and home.

Our team of skilled nurses, physicians and therapists will design a plan of care for you that is multidisciplinary, comprehensive and goal-oriented.  Upon admission, the team will assess you.  Your caregivers will continue to reassess you and work with you to design your discharge plan.

We recognize that family support promotes recovery and contributes to improved outcomes.  We encourage your loved ones to participate in care conferences.  We will also work with you about your condition and how to best manage when you leave Exeter Healthcare.
