Medical Insurance - pre tax
Our medical insurance plan has been carefully designed to preserve your freedom of choice while managing overall costs. With this three-tier approach you decide where to receive your care - inside or outside of our EHR-Affiliated (EHRA) Network (Tier 1) which allows you to control your out-of-pocket costs.
Wellness Credit Incentive Program
This voluntary program encourages employees enrolled in our health insurance plan to attain certain Wellness targets so they may lead healthier lives. Attaining these targets qualifies employees for a per pay period discount off the cost of your health insurance premium. Discounts are also available to covered spouses who attain Wellness targets.
Dental Insurance - pre tax
100% coverage for routine diagnostic and preventative care; 80% coverage for minor restorative services and 50% coverage for major restorative coverage. Minor and major restorative coverage requires a $25 per person annual deductible. (Eligible at 56hrs per pay period.)
Vision Discount Program:
This program is offered in conjunction with the dental plan and gives you access to over 30,000 vision care providers (including optometrists, ophthalmologists, and opticians) nationwide at 16,000 locations. This program is accepted at the nation's leading optical retailers including Lens Crafters, Pearle Vision, Sears Optical and Target Optical.
Basic Life and AD&D coverage at no cost to you, in an amount approximately equal to your annual salary. You may choose to purchase additional coverage (Supplemental Life and Supplemental AD&D) equal to one, two, or three times your basic coverage. In the event you leave the organization, you may be able to convert your Basic Life or Supplemental Life coverage to an individual plan at non-group rates, or at preferred rates with evidence of good health. (Eligible at 40 hrs per pay period)
Dependent Life Insurance
This coverage is available for legal spouses and children to age 26. Employees can purchase coverage for legal spouse only, child/children only, or a family combination of legal spouse and child or children. Employees pay the same cost to cover all of their eligible children regardless of how many they have. In the event you leave the organization, you may be able to convert the Dependent Life coverage to an individual plan at non-group rates, or at preferred rates with evidence of good health.
Short Term Disability & Long Term Disability
Short term disability insurance is designed to replace 60% of your weekly salary, tax free, to a stated maximum per week for up to 24 weeks with the first 14 days as a waiting period. Long term disability insurance provides coverage protection for disabilities that extend beyond 180 days. You may choose from different levels of coverage. (Eligible at 56hrs per pay period)
Retirement options - pre tax
Options include Pension, 403B/457B/ROTH. (Eligibility varies depending on affiliate and program)
Earned Time (Vacation, holiday, and sick time plan)
Earned time accruals will be calculated on the following pay categories: regular time, earned time (excluding earned time payout), bereavement, and jury duty. The sum of these hours (not to exceed 80 hours per pay period) will be multiplied by the earned time accrual factor. There are length-of-service increases. You can pre-elect to cash in earned time once per year that will be earned in the upcoming year.
Employees may carry over 480 hours of earned time from year to year. Any amount of earned time in excess of 480 hours will be forfeited.
Flexible Spending - pre tax
Flexible Spending Accounts allow you to redirect a portion of your salary, on a pre-tax basis, to pay for qualified, unreimbursed medical or dependent care expenses incurred during that plan year. Once FSA's have begun, only certain family status changes will permit you to change or stop your deduction. The annual maximum which may be redirected is $2,500 for medical and $5,000 for dependent care. NOTE: The IRS requires the forfeiture of unused account balances. (Eligible at 40hrs per pay period.)
MetPay is available to employees who work at least 40 hours per pay period. Through MetPay employees can obtain auto, homeowner's, or renter's insurance at group rates. The premium is paid through payroll deduction.
FINANCIAL ASSISTANCE is available for undergraduate and graduate courses, which are job-related or lead to a job-related degree. You must have approval from your supervisor and the Education Department prior to beginning your course. To receive tuition assistance, you must complete the course with a grade of "C" or better. Percentage based on number of hours worked and on affiliate participation.
Cell Phones and Pagers: Employees may purchase cell phones at a discounted rate through AT&T and Verizon Wireless.
Dell Purchase Program: Due to our affiliation with Dell Computers as one of our premier vendors, employees of our organization are now entitled to participate in Dell's Member Purchase program and limited time special offers.

EAP (Healthwatch Employee Assistance Program) is designed to provide confidential assistance to employees who are experiencing problems related to family/marital discord, emotional disturbances, financial difficulties, stress management issues, alcoholism, chemical dependency, etc. This benefit is free to employees and their household members.
Food & Nutrition: Our Food & Nutrition department offers a variety of bulk and single food items -- from boxed hamburgers, hot dogs, and chicken, to cakes for special occasions -- at a discounted rate to employees.
